CareCubed to be used in greater depth especially at care planning stage (pre-placement) and review stage (during placement).Work with Head of Sufficiency and operational teams to develop a robust dynamic approach to assessment/care planning/use of CareCubed (pre-placement) and reviews during placement to accelerate opportunities for step down across several care pathways and maximise current market sufficiency and improve value for money.Work with the Head of Sufficiency role, service leads and strategic commissioners to develop forward looking needs analysis across several pathways leading to and out from high cost placements to ensure right care is provided at each step of the individual's journey.Work with the Head of Sufficiency role and strategic commissioners to stimulate, develop and shape the local market and build strong relationships with providers to improve our ability to place effectively and improve value for money.Interview date: 27/08/#####We offer a wide range of
