As a Commissioning Broker, you will be at the heart of connecting people with the care they need. Acting as the link between families, professionals, and providers, you will help secure high-quality care packages and placements that truly make a difference. In this role, you will: Support safe and timely hospital discharges, working closely with partners to ensure everything is in place Manage a diverse and sometimes complex caseload Maintain accurate and up-to-date records across systems Handle sensitive information with professionalism and confidentiality Monitor provider performance to maintain high standards of care Work collaboratively with managers to improve processes and outcomes This is a fast-paced, meaningful role where your work has a direct impact on people's lives every day., If you hold the same behaviours and approach, then we want to hear from you!
