Bilingual Dementia care navigator / advisor

Work with individuals and their families, to provide information and signposting and onward referrals to support the person to live as best they can with the illness. This may include referrals to the carers centre, Adult Social Care, telecare, among others. We work closely with the Alzheimer Society who run social inclusion groups in the borough. Working with the patient and their carers on a personalised care plan and reviewing before discharging back to the GP. To facilitate Bengali Cognitive Stimulation Group. • NVQ Level 3 qualification to diploma equivalent level or above

• Qualification in health or social care related subject
  
  Experience of working with people who have a disability and/or physical/mental health/sensory difficulties
• Experience of using Excel software for data recording and reporting
• Experience of team working
• Experience of co-facilitating and presenting information to groups
• Experience of working with people who have a learning disability, mental health issues and behaviours which may challenge
• Experience of working with people who have communication difficulties or are non-verbal
• Experience of working with paid and unpaid carers
