Are you an Occupational Therapist who thrives in a fast-paced environment and enjoys making a real difference when patients need support most? Join our Urgent Community Response (UCR) Team, a dynamic and highly skilled multidisciplinary team providing rapid assessment and intervention across Barking & Dagenham, Havering and Redbridge. Our service responds to physical health crises in patients' own homes, helping people avoid unnecessary hospital admissions and supporting them to remain independent within their communities. This is an exciting opportunity for an enthusiastic and motivated Occupational Therapist who enjoys autonomous working, complex clinical decision-making and collaborative multidisciplinary practice., As part of our UCR team, you will undertake urgent assessments within patients' homes, delivering timely interventions that support recovery, reduce hospital admissions and promote independence. You will need strong clinical reasoning, excellent communication skills and the confidence to make decisions in often complex and rapidly changing situations. No two days are the same, and you'll be supported by an experienced MDT that values teamwork, innovation and shared learning. We are particularly interested in candidates who are passionate about frailty, urgent care and falls management. You will regularly assess patients who have experienced falls and may remain on the floor for extended periods, requiring a compassionate, holistic and proactive approach. Curiosity, initiative and a willingness to look beyond the immediate presentation are highly valued within our team.
