Full Job Description
Oxleas NHS Foundation Trust is seeking an experienced and motivated Registered Nurse to join the Primary Care team at HMP Bristol on a bank basis. This is an opportunity for a nurse with a strong background in practice nursing, district nursing, long-term condition management, wound care and physical health to provide high-quality, evidence-based primary care to people in prison. The successful candidate will complement the existing nursing workforce and provide additional specialist primary care capacity, supporting the delivery of care that is equitable with that available in the community. Working within a multidisciplinary healthcare team, you will undertake assessment, treatment, monitoring and review of patients with a range of acute and long-term health conditions. You will be expected to work autonomously within your scope of practice, while recognising when escalation or multidisciplinary input is required., The post holder will:o Undertake comprehensive nursing assessments and develop, implement and evaluate individualised care plans.o Provide evidence-based management and review of patients with long-term conditions.o Undertake chronic disease reviews and contribute to the management of patients with complex physical health needs.o Support the management of conditions including diabetes, cardiovascular disease and hypertension, respiratory disease including asthma and COPD, epilepsy, chronic kidney disease and other long-term and complex physical health conditions.o Undertake assessment and management of wounds, including chronic wounds and leg ulcers.o Provide leg ulcer care, wound assessment, dressing selection and application, and compression therapy where appropriately trained and competent.o Recognise deterioration in patients with long-term conditions and initiate appropriate escalation and referral.o Undertake physical health observations, clinical assessment and appropriate investigations within scope of practice.o Provide health promotion, prevention and patient education.o Support screening and preventative healthcare programmes.o Contribute to medication monitoring and medicines optimisation within professional competence and local procedures.o Liaise effectively with GPs, advanced practitioners, pharmacy, specialist nurses and other members of the multidisciplinary team.
In order to work as a bank only worker you will need to have completed preceptorship training or obtained 6 months post qualification experience.
