Full Job Description
- Provide person-centred support to individuals in their homes and community settings.
- Assist with daily living activities, from attending appointments to accessing community groups.
- Offer emotional support and encouragement, helping people build resilience and confidence.
- Work closely with healthcare professionals, social workers, and voluntary organisations to coordinate care.
- Keep accurate records and contribute to care plans that reflect each person's unique needs and goals.
- Promote healthy lifestyle choices and connect people to wellbeing initiatives.

- Strong communication and listening skills, with the ability to build trust quickly.
- An understanding of mental health, safeguarding, confidentiality, and professional boundaries.
- Flexibility, empathy, and a problem-solving mindset.
- A valid driving licence (desirable but not essential).
