Bank Healthcare Assistant

The applicant will be responsible for visiting individuals in their own home managing personal care tasks and medication.,

• Deliver personal care and assistance with daily living to recently discharged patients
• Support medication administration and accurate documentation
• Observe, record, and escalate changes in condition following care plans
• Assist with mobility, nutrition, and other essential care needs
• Communicate effectively with service users, families, NHS staff, and social care teams
• Maintain accurate records using our electronic care record system
• Adhere to infection control, safeguarding, and health & safety procedures
  
  We are seeking experienced and compassionate carers to join our Post Hospital Bridging Service across East Sussex. This important role helps relieve NHS bed pressures by providing interim home-based care for patients discharged from hospital awaiting social care assessments. You'll work independently in the community, delivering high quality, person centred care that supports independence, dignity, and comfort during patients transition from hospital to home., Right to work and live in the UK (UK Citizen or Indefinite Leave to Remain)
