Full Job Description
In this role, you will plan, organise, deliver, and evaluate an effective and evidence-based health visiting service, ensuring that children and families receive the highest standard of care. Working within a multidisciplinary team, you will provide skilled health visiting interventions in a variety of community settings, promoting positive health outcomes and supporting families to achieve the best possible start in life. You will work collaboratively with healthcare professionals, partner agencies, and local services to ensure seamless and integrated care for children, families, and communities. As part of the wider team, you will contribute to the achievement of Quality Strategy objectives and support the continuous improvement of services. The successful candidate will demonstrate excellent clinical skills, strong communication and leadership abilities, and a commitment to delivering safe, effective, and person-centred care. You will also support the development of colleagues through training, teaching, and professional leadership, helping to maintain high standards of practice across the service.,
- Deliver the healthy child programme and work in partnership with families to develop and agree tailored health plans as required
- Utilising specialist skills and knowledge, advise and support individuals and families on evidence-based health care practices.
- Understand the caseload profile including the health needs of the defined population and local communities, targeting outreach services to meet their needs.
- Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children
- Assess, plan, implement and evaluate individual/group programmes of care that meet the public health needs of the population
