Bank GP - Intermediate Care and Hospital at Home (Virtual Ward)

We are seeking Bank GPs to support our Intermediate Care and Hospital at Home services. This is an exciting opportunity to join a developing GP-led model that supports people to receive treatment closer to home. Flexible bank opportunities are available across bed-based, community and virtual pathways. The Stamford Unit is a 96-bed Intermediate Care facility providing rehabilitation, end-of-life care, step-up admissions and complex discharge support. You will provide medical care for patients, undertake ward rounds and work alongside ACPs, Nurses, Therapists and Social Workers to deliver coordinated care. Our Hospital at Home service provides hospital-level care in patients' homes through remote monitoring, telemedicine and multidisciplinary support. Working with Urgent Community Response teams, advanced practitioners and specialist services, you will undertake clinical assessments, support admission avoidance and early discharge, and contribute to personalised care planning. This varied role combines community-based acute care and digital healthcare, offering opportunities to develop interests in frailty, older people's medicine, palliative care and telemedicine while helping deliver innovative models of care across Tameside and Glossop.,

• Provide clinical leadership across Intermediate Care, Hospital at Home and Virtual Ward services, ensuring safe, effective and high-quality care.
• Undertake assessment, diagnosis, treatment planning and ongoing management across bed-based, community and virtual pathways.
• Work with multidisciplinary teams to deliver coordinated, person-centred care and support treatment, escalation and discharge planning.
• Deliver clinical care through ward rounds, home visits and telemedicine consultations.
• Support admission avoidance, early discharge and care closer to home through Hospital at Home and Virtual Ward pathways.
• Manage patients with frailty, rehabilitation, chronic disease, palliative and acute care needs in line with best practice.
• Work closely with primary care, community services, care homes and system partners to ensure seamless care.
• Contribute to service development, pathway improvement, innovation and the promotion of best practice.
• Participate in clinical governance activities including audit, incident reviews, complaints, patient safety investigations and quality improvement.
• Work flexibly across services to meet patient and operational needs.
• Support the development of colleagues and act as a role model for professionalism and clinical excellence.
