Full Job Description
We are looking for experienced Community Health & Social Care Support Workers (Band 3) to join our bank and support services across Derbyshire. You will work within small community support bed and assessment units, providing high-quality, person-centred care to adults with a range of physical, mental health and social care needs. This is a bank / ad-hoc role offering flexible shifts rather than guaranteed full-time hours.,
- Provide compassionate, person-centred care in line with individual care plans.
- Support with personal care, mobility, eating and drinking, and activities of daily living.
- Complete basic observations (e.g. blood pressure, blood glucose, urinalysis) and escalate concerns appropriately.
- Promote independence, dignity and wellbeing, including social and emotional support.
- Work as part of a multi-disciplinary team, following agreed policies, protocols and safeguarding procedures.
- Maintain accurate records and contribute to safe, effective service delivery.
Minimum 1 year of UK experience as a Healthcare Assistant / Support Worker in a health or social care setting. - Experience working with adults with physical disabilities, learning disabilities, mental health needs and/or older adults.
- Ability to travel reliably to at least one of the three sites (Buxton, Ilkeston, Staveley) for early, late and night shifts.
- Good understanding of safeguarding, confidentiality and professional boundaries.
- GCSE Maths and English (or equivalent) and one of:
- NVQ Level 3 in Health & Social Care, or
- Equivalent health/social care qualification, or
- Equivalent proven experience in a CQC-regulated organisation with Care Certificate evidence. Compliance
- Right to work in the UK (we cannot provide sponsorship for this role).
- DBS (preferably on the Update Service).
- Able to provide references covering the last 3 years.
- Willing/able to provide immunization evidence as required (e.g. TB, Hep B, Varicella, MMR).
