Full Job Description

The Specialist Community & Crisis Support service provides 24-hour specialist advice, support and facilitation of care for those persons with complex symptom management due life limiting illness and support to carers of the individual within the home setting, at a nurse led clinic environment or within the office on telephone support.

Our Referral Hub receives all referrals for individuals being referred to the hospice services, assessing and identifying the specialist interventions required, Fixed Bank Salary of £47,665 per annum, pro rata (£24.38 per hour)

To work collaboratively with the palliative care team and primary care teams providing high quality specialist support, advice and symptom control to Individuals with specialist complex palliative care needs and their carers in their primary care setting.

To provide specialist telephone assessment and support for healthcare professionals, Individuals and relatives as required via the Specialist Advice Line, whilst documenting with accuracy.

To work both autonomously and a team member to provide a specialist resource in palliative care and work in close collaboration with acute hospital and other specialist palliative care services and the wider multidisciplinary team.

To provide specialist support, decision-making skills, expert advice/consultancy and education to members of the primary care team to support them with their management and care of people with life limiting illness in the community.

To provide rapid crisis access to specialist advice for professionals, persons in the community and those involved in their care to crisis telephone support, assessment, decision making, prioritisation of need and face to face crisis visiting as appropriate.

An opportunity has arisen for a dynamic, enthusiastic, self-motivated, experienced nurse with good clinical palliative care knowledge base, to join our well-established specialist community services as a Clinical Nurse Specialist (Bank). This post will provide varied senior experience in a multidisciplinary working environment delivering a 24/7 service to individuals, carers and other health care professionals in the community.

We are looking for a candidate who is working at a senior lever and has excellent communication, assessment and prioritisation skills, and understands the needs of those receiving palliative care /end of life care, a working knowledge of evidence -based practice and the ability to develop a good knowledge base of current initiatives and developments in palliative care.

The Service works within the boundaries of Barking & Dagenham, Havering, and Brentwood and provides out of hours advisory telephone support to Redbridge.

We are a dynamic, forward thinking organisation delivering a full range of specialist palliative care services for individuals & their loved ones with life limiting illness. We achieved Outstanding in our CQC inspection & have reconfigured our services to meet the changing needs of our local community. We support 5 Clinical Commissioning Groups & have a robust governance structure in place. We provide ward clinical services within a modern, personal & friendly 18-bedded unit, as well as Specialist Community Crisis Support Service, Hospice at Home Service & Specialist Day Therapy Service. We also have the hospice Pepperell Education Centre that has an excellent reputation for delivering high quality education programmes and study days.
