Full Job Description

The post holder will assist in the supportive interventions for young people who are experiencing a crisis. They will also provide support, guidance and psycho education to care givers as part of an 'empowering parents' approach. The support worker will provide this care under the direction of the Children Crisis Assessment & Treatment Team (C-CATT) Multidisciplinary Team (MDT). Due to the nature of the role, the support worker will regularly work alone without direct supervision for part of the span of duty and will be required to travel across the county to see young people and families in the community., Update the C-CATT crisis coordinator and MDT of the young person's progress and observations of changes in their mental health and ensure these changes are documented on the electronic record

Regularly meet with supervisor and/or line manager to ensure the post holder is supported to carry out the demands of the post

Ensure that all care services provided are offered without prejudice regardless of age, creed, gender and ethnicity

Be aware of diverse cultural and ethnic needs and facilitate appropriate support and care

The post holder will also have the opportunity to assist with and/or run both virtual and face to face group sessions on topics like self-harm / safety at home / mindfulness.

HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving
