Full Job Description
We have a fantastic opportunity for a Advanced Clinical Practitioner to provide clinical leadership within a community-based meeting of Multidisciplinary Team (MDT) Vascular clinic across the Oldham area on a 12 months fixed term contract. This service will consist of patients who have been referred by their GP due suspected peripheral arterial disease (PAD).The patient journey will require a comprehensive vascular assessment, diagnosis (including ABPI) and a management plan, which may include a diabetic review +/- medication changes, lifestyle advise / health promotion and onward referrals where appropriate. The successful candidate will be an experienced Advanced Clinical Practitioner with expertise in diabetes vascular care. You will have a genuine passion for diabetes management and a commitment to delivering excellent patient-centred care within a primary care setting. Variety of shifts available, Monday to Friday between 08:00 and 18:30.,
- Mobilisation of the well leg clinic, developing clinical pathways, standard operating procedures and assessment templates.
- Supporting with training and undertaking of comprehensive assessments, diagnosis and management of PAD and diabetes management.
- Providing holistic care to patients with diabetes, including monitoring, education, and support
- Leading, supporting and training junior staff with the MDT clinics and innovative ways in meeting individual patient needs (focussing on those that are underserved).
- Collaborating with GPs, multidisciplinary teams and local services to optimise patient outcomes
- Promoting best practice in line within the current guidelines
- Providing data and analysis on specific patient outcomes and key performance indicators.
