Full Job Description
Due to expansion of the Haematology Clinical Nurse Specialist Team, we have an exciting opportunity for an experienced Haematology nurse to join us as a Lymphoma CNS. The post-holder will work alongside the existing Haematology CNS team to deliver holistic care to patients diagnosed with Lymphoma, from diagnosis to end of treatment or end of life. UHB have successfully implemented Patient Stratified Follow Up for Lymphoma patients, and the CNS will ensure the continued provision and success of this service. The post-holder will be required to complete Non-Medical Prescribing, if not already held, and will deliver nurse led clinics.,
- Act as a key worker for patients diagnosed with Lymphoma
- Provide holistic support throughout patient pathway, from diagnosis to end of treatment or end of life
- Provide expert, specialist advice to patients and their loved ones
- Manage own caseload and run nurse led clinics
- Manage PSFU service alongside existing CNS
- Contribute to audit activity, service development, pathway improvement and the nursing education programme within Lymphoma
- Meet new patients at diagnosis and provide verbal and written information on the disease and treatment, and provide CNS contact details
- Ensure patients are booked for treatment or tests in a timely manner e.g. SACT treatment, bone marrow biopsy, blood tests, inpatient admissions
- Ensure patients understand treatment toxicities and know when to seek advice
- Undertake Holistic Needs Assessments to ensure patients have ongoing personalised support throughout and at end of treatment
- Support consultant led clinics ensuring patients have access to their CNS during or after clinic appointments to discuss any concerns or worries, and to provide further information if required
- Review patients on the inpatient wards and in the Centre for Clinical Haematology Day Unit to ensure ongoing CNS support
- Provide telephone support to patients
- Liaise with the Bone Marrow Transplant team for patients proceeding to transplant to ensure a smooth handover of care
- Refer to other teams and agencies as appropriate e.g. psychology support, financial support
Applicants must have recent experience working in Haematology as a Clinical Nurse Specialist at Band 6, with a strong understanding of Systemic Anti-Cancer Treatment and associated toxicities. An accredited Health Assessment qualification is essential for this role. Applicants will be a confident, proactive and compassionate practitioner with strong haemato-oncology experience and excellent communication skills. They should be able to work autonomously, prioritise effectively and demonstrate advanced clinical decision-making.
