Full Job Description
Are you an experienced Cardiology Nurse passionate about improving the lives of people living with heart failure? Ready to make a real difference by delivering specialist care closer to home? Central London Community Healthcare NHS Trust (CLCH) has an exciting opportunity for a Band 7 Community Heart Failure Specialist Nurse to join our Brent and Harrow cardiology service on a 12-month fixed-term, part-time basis. Working autonomously within allocated Primary Care Networks, you will manage a specialist caseload of patients with complex heart failure needs. You'll deliver comprehensive assessments, personalised treatment plans and evidence-based interventions, helping patients manage symptoms, optimise treatment and avoid unnecessary hospital admissions. You'll work closely with GPs, consultant cardiologists and multidisciplinary colleagues, while supporting patients and carers with education and self-management. You'll also contribute to clinical audit, quality improvement and service development. Supported by an experienced Clinical Service Lead and Cardiology Clinical Specialist, both with advanced practice qualifications, you'll have opportunities to further develop your expertise. A full UK driving licence and access to a car are required, subject to the exception set out in the person specification. Join CLCH and help deliver outstanding heart failure care in the community! This is a varied and rewarding role combining specialist clinical practice, leadership, education and service development. Key responsibilities include:
- Manage a specialist caseload of patients with complex heart failure needs, providing comprehensive assessments, individualised treatment plans and evidence-based care.
- Initiate, titrate and optimise heart failure therapies, supporting symptom management, patient self-care and the prevention of avoidable hospital admissions.
- Work collaboratively with GPs, consultant cardiologists, Primary Care Networks (PCNs) and multidisciplinary teams to ensure coordinated and effective patient care.
- Provide specialist clinical advice, education and support to patients, carers and healthcare professionals across community services.
- Demonstrate clinical leadership through mentorship, supervision and the delivery of training to support workforce development.
- Contribute to the development and implementation of clinical guidelines, care pathways and service improvement initiatives.
- Participate in clinical governance, audits, research and quality improvement projects to promote patient safety and evidence-based practice.
- Support the ongoing development of the Community Heart Failure Service, working closely with the Clinical Service Lead to improve patient outcomes and service delivery.
