Full Job Description

We are offering an opportunity for an experienced Children's Nurse to join our Children's Community Nursing Team as a Band 7 Palliative Care Nurse. This role is part of the Kentown Support investment in Greater Manchester, strengthening community-based palliative and end-of-life care for babies, children and young people.
Fully funded through the Kentown Support programme, with recurrent NHS GM funding secured, this post supports a coordinated and equitable model of care. You will provide specialist assessment, clinical leadership and high-quality palliative care, working closely with families, community teams, acute services and children's hospices.
Nurses will work within locality community teams in six GM areas without specialist provision, using an in-reach and outreach model supported by a cluster approach for resilience and peer support. A phased 24/7 community PEOLC offer will follow.
The team is supported by a Band 8a Clinical Lead and the Kentown Support Head of Programmes. All staff attend bimonthly team days for reflection, problem-solving and CPD. Development opportunities include a 6-month rotation with the Alder Hey Specialist Palliative Care Team and funded CPD.
We are seeking compassionate, motivated nurses who can lead, collaborate and help shape this new model of care, making a meaningful difference to children and families across Greater Manchester., As a Band 7 Children and Young People's Palliative Care Nurse, you will deliver high-quality palliative and end-of-life care across Greater Manchester.
You will provide specialist nursing care in homes, community settings and through in-reach to acute trusts, including assessments, symptom management, advance care planning and regular review. You will offer compassionate, family-centred support and promote preferred place of care and death.
Working within a locality Children's Community Nursing Team, you will deliver care through an in-reach/outreach model and support the development of a consistent 24/7 community PEOLC offer. You will collaborate with families, community teams, acute services, hospices and voluntary sector partners, and work with Kentown Support colleagues to provide wrap-around support. Participation in a cluster model will strengthen resilience and peer support.
You will provide clinical leadership, support colleagues' development and contribute to service evaluation and improvement. Attendance at bimonthly Kentown Support Team days is required for reflection, problem-solving and CPD. You will also deliver education and signposting for professionals, parents and carers., The Band 7 Children & Young People's Palliative Care Nurse will play a key role in delivering the Kentown Support model across Greater Manchester, providing specialist community-based palliative and end-of-life care for babies, children and young people with life-limiting and life-threatening conditions. The postholder will work within a locality Children's Community Nursing Team while contributing to a pan-GM integrated model of care designed to improve equity, coordination and quality of support for families.
The role combines expert clinical practice, service coordination, leadership, education and partnership working across health, social care and voluntary sector organisations.
Main Responsibilities
1. Specialist Clinical Practice
+ Provide high-quality, holistic palliative and end-of-life care to children and young people in their homes, community settings and through in-reach to acute hospitals.
+ Undertake comprehensive assessments, symptom management, advance care planning and ongoing review of children with complex and life-limiting conditions.
+ Support families to understand their child's condition, treatment options and care plans, ensuring compassionate, family-centred communication.
+ Facilitate preferred place of care and preferred place of death wherever possible.
+ Respond to urgent and unplanned care needs, working flexibly to support children and families during periods of deterioration or crisis.
2. Integrated Working Across Greater Manchester
+ Work as part of a locality Children's Community Nursing Team in one of the six GM localities currently without specialist paediatric palliative care provision (Wigan, Bolton, Tameside, Stockport, Bury or Salford).
+ Deliver care using an in-reach and outreach model into local acute trusts to ensure continuity and coordination across settings.
+ Contribute to the development of a consistent, equitable 24/7 community-based PEOLC offer across GM, including participation in the phased development of an out-of-hours service.
+ Work collaboratively with children's hospices, acute paediatric teams, community services, GPs and voluntary sector partners.
3. Coordination and Partnership Working
+ Act as a key point of contact for families, coordinating care across multiple agencies and ensuring timely communication between professionals.
+ Work alongside Kentown Support family support workers, coordinators, counsellors and bereavement teams to provide wrap-around support.
+ Participate in a cluster model with other Kentown nurses to promote resilience, peer support, shared learning and cross-cover for sickness and annual leave.
+ Attend multi-agency meetings, case discussions and safeguarding reviews as required.
4. Leadership, Education and Service Development
+ Provide clinical leadership within the locality team, modelling best practice in paediatric palliative care.
+ Support colleagues to develop their skills and confidence in caring for children with life-limiting conditions.
+ Contribute to the evaluation and continuous improvement of the Kentown Support model, including data collection, audit and feedback.
+ Participate in bimonthly Kentown Support Team days to review operational delivery, problem-solve, reflect and contribute to service development.
+ Deliver training, education and signposting for professionals, parents and carers in line with Kentown Support's vision to equip the wider system.
5. Professional Development
+ Engage in a structured programme of training and competency development in paediatric palliative and end-of-life care.
+ Participate in a 6-month rotation with the Alder Hey Specialist Palliative Care Team to enhance specialist knowledge and leadership capability.
+ Access funded CPD opportunities, including the Advanced Practice in Palliative and End of Life Care for Children and Young People module at the University of Liverpool.
+ Maintain up-to-date knowledge of national guidance, best practice and emerging evidence in paediatric palliative care.
6. Governance, Safety and Quality
+ Ensure care is delivered in line with national standards, local policies and evidence-based best practice.
+ Contribute to safeguarding processes, risk assessments, incident reporting and quality improvement initiatives.
+ Maintain accurate, timely and holistic documentation across all care settings.
+ Participate in clinical supervision, reflective practice and peer review to support safe, high-quality care.
7. Professional Conduct

+ Demonstrate compassion, professionalism and emotional resilience when supporting families during highly sensitive and distressing situations.
+ Work flexibly to meet the needs of children and families, including travel across Greater Manchester.
