Full Job Description

You will work autonomously while being part of a wider multidisciplinary team, managing a varied caseload, completing comprehensive assessments and developing personalised care plans that promote independence, choice and self-management. You will work closely with patients, families, carers, GPs and other health and social care professionals to coordinate care and help prevent avoidable hospital admissions. You will also have the opportunity to develop your leadership skills by supporting junior colleagues and students, contributing to service improvement and helping us deliver consistently high standards of care.   • Work autonomously to assess, plan, deliver and evaluate high-quality, person-centred nursing care for patients with a range of complex and long-term health needs.
  • Manage and coordinate a caseload, taking a proactive approach to supporting patients in the community and helping to prevent avoidable hospital admissions.
  • Complete holistic assessments that consider patients' physical, psychological and social care needs, including the needs of their families and carers.
  • Work collaboratively with GPs, community nurses, operational leads and other health and social care professionals to identify and support patients who would benefit from proactive case management.
  • Advocate for patients, ensuring their individual needs, preferences and choices are at the heart of their care while promoting independence and self-management wherever possible.
  • Recognise and respond appropriately to changes in a patient's condition, maintaining accurate records and escalating concerns when required.
  • Coordinate care and make appropriate referrals to other services to ensure patients and carers receive the right support at the right time.
  • Provide clear information, advice and support to patients, families and carers, empowering them to make informed decisions about their health and wellbeing.
