Full Job Description
+ To work as part of a team ensuring all referrals are triaged and have a timely visit scheduled.
+ To work as part of a team ensuring all new admissions to the Community Nursing services have a clinically robust assessment and patient focused care plan.
+ This is borough wide cover.
+ To introduce and enable self management and self care.
+ To work as part of a team ensuring all patients have a safe handover to the professional and team to complete the care plan and measure the outcomes.
+ To work in partnership with the locality teams and partner providers to maintain effective patient flow.
+ To work within the financial resources for the patients and the service.
+ To work shifts patters on a rota: (8am-4pm, 10am-6pm and 12pm-8pm) Monday to Sunday., As part of the relocation scheme we presently have, the Trust could support you with offering up to £10,000 for removal and associated expenses. This is subject to change and terms and conditions apply.

We in the District Nursing team, have an ambitious vision for the Tower Hamlets community health service. Our Community
We provide a wide range of community health and mental health inpatient services to children, young people, adults of working age, and older adults in The City of London, Hackney, Newham, Tower Hamlets, Bedfordshire and Luton. We also provide psychological therapy services to the London Borough of Richmond, and provide Forensic services to the whole of North East London. We provide services in urban and rural settings, sometimes in our inpatient units, but mostly in the community close to where people live. Integrated care is key in making the best use of resources and providing effective care.
