Are you looking for a new challenge? East of England Community Health and Care NHS Trust are looking for a Registered Band 6 Physiotherapist to join the Inpatient Community Therapy Team based at North Walsham Hospital., The post is based at North Walsham Memorial Hospital. You will work as a member of the therapy team on the ward where you will work alongside the registered Physiotherapy and Occupational Therapists, Assistant Practitioners and Therapy Healthcare assistants. The main duties include:
- Delivery of high quality physiotherapeutic interventions and health care to patients . To provide proactive, holistic case management and rehabilitation whilst working with patients to help them set realistic goals. Utilising health coaching skills to effectively engage with the patient and their family to ensure that expectations are appropriately managed.
- To assess and prescribe care programmes for patients with, but not exclusive to, long term conditions, the frail and elderly, palliative care and rehabilitation needs, to achieve quality of life and independence where possible
- To assess and prescribe exercise programmes to patients with, but not exclusive to, frail and elderly, palliative care and rehabilitation needs, to achieve quality and independence where possible.
- To work within the integrated team to facilitate early discharge from hospital
- To work with all health care professionals, and statutory and non-statutory agencies to provide a seamless integrated service to our service users
- To provide advise, support and supervision to junior staff, therapy assistants and students.

You must be self-motivated, able to demonstrate sound clinical reasoning skills and have good communication skills.
Based at North Walsham Hospital Inpatient Unit you will work with a small team of friendly, approachable physiotherapists, occupational therapists, therapy assistant practitioners as well as registered nurses and healthcare assistants. The 24 bed inpatient rehabilitation unit provides admission avoidance and supported discharge using an integrated approach to care and rehabilitation through close multidisciplinary working.
