Band 6 Occupational Therapist - Community Falls Team

Are you an Occupational Therapist looking to develop your knowledge and skills working in the community?

We are a friendly, versatile and supportive community multi disciplinary team based within Solihull Hospital. Our patients all have a Solihull GP, and all have a risk or history of falls, often alongside complex vulnerabilities and health and social issues. You will hold an interesting caseload of patients, working with them and their carers and relatives to reduce their risk of falls, ultimately to ensure their goals are met and they remain living safely in the community with reduced risk in the future.

Your role will include carrying out face to face holistic, multifactorial falls assessments within patients homes and care homes, before linking with the team, other community colleagues and the wider BSOL network to work together to reduce the future risk of falls for each patient. Our remit is large and we signpost and involve many different agencies to ensure that we address all identified risks as far as we can.

You will also be involved in triaging referrals, conducting telephone assessments, supporting students and junior staff, providing and attending in service training, and assisting with audit, research and leadership., To provide a high standard of Occupational Therapy within the Specialist Community Falls Team across the borough of Solihull.

To undertake holistic multi-disciplinary (multi-factorial) assessments and provide relevant falls prevention interventions with patients in their own homes with the aim to reduce patient falls and hospital admission rates.

To take on a triage and co-ordinating role within the team.

To contribute to seven day working programmes across the organisation.

To contribute to the development of the Falls service including involvement in ongoing projects.

To take an active role in supporting students and new starters of all levels and disciplines

To be involved in new and developing initiatives within the team and wider Community Therapies within UHB
