Band 6 Mental Health Practitioner

You'll manage a caseload of people with severe and enduring mental health needs, providing assessment, care coordination and evidence-based interventions. You'll work autonomously while contributing to a multidisciplinary team, supporting junior colleagues and helping shape high-quality community mental health care. Opportunities for supervision, professional development and service improvement., As a Band 6 Mental Health Practitioner in CMHS, you will play a role in delivering high-quality, recovery-focused care to people with severe and enduring mental health needs in the community. Your experience and clinical judgement will make a real difference to service users, carers and colleagues You will: Manage a caseload of people with complex mental health needs, taking responsibility for care coordination and treatment. Undertake comprehensive assessments and develop, implement and review evidence-based care plans. Assess and manage risk, including responding effectively to crisis and urgent situations. Provide individual interventions and support people towards their recovery goals. Participate in triage and assessment of new referrals into the CMHT. Work autonomously while contributing to a multidisciplinary team. Provide clinical advice, consultation and support to junior colleagues, including during relapse or crisis. Supervise junior multidisciplinary staff and support their professional development. Deputise for the Clinical Team Leader when required. Build effective relationships with service users, carers, families and partner agencies. Contribute to audit, service evaluation, quality improvement and service development. Participate in teaching, mentoring and practice development. Undertake duty functions and community appointments as required. • RN [MH] with live NMC registration or OT or Social worker with current professional registration.

• Evidence of undertaking audit work.
• Innovative, creative and a willingness to try new ideas.
