Full Job Description

You will work with adults with severe and enduring mental health difficulties, delivering highquality assessment and evidencebased psychosocial interventions using a recoveryfocused and strengthsbased approach. As a senior practitioner within the team, you will contribute to clinical leadership, provide supervision to colleagues, and work collaboratively with a wide range of partner agencies across health, social care, voluntary and community services. The role requires flexibility, autonomy and the ability to manage complex clinical situations, including risk assessment and safeguarding. A full driving licence and access to a vehicle for business use are essential. Main duties of the job

  • Manage a caseload of service users with severe and enduring mental health needs within a community setting.
  • Undertake comprehensive mental health assessments, risk assessments and care planning using a recovery and strengthsbased approach.
  • Deliver evidencebased psychosocial and therapeutic interventions, drawing on knowledge of psychological therapies and recovery models.
  • Assess and manage risk, including safeguarding adults and children, escalating concerns appropriately and participating in statutory processes where required.
  • Work collaboratively with service users, carers, families and a wide range of partner agencies to promote recovery and wellbeing.
  • Contribute to clinical leadership within the team, including supervising Band 5 and unregistered staff and supporting students.
  • Deputise for the Team Manager when required, assisting with triage, workload allocation and team coordination.

As a senior member of the team, you will rolemodel high professional standards, provide supervision and mentorship, contribute to service development, clinical audit and quality improvement, and support the ongoing development of a recoveryfocused service. You will liaise closely with GPs, inpatient services, voluntary sector partners, social care, housing and other agencies to ensure coordinated and effective care.     We are seeking an experienced and motivated Band 6 Mental Health Practitioner to join our Community Mental Health Service based at Glanville House, Bridgwater. This role is open to Registered Mental Health Nurses, Occupational Therapists or Social Workers with significant postregistration experience in community mental health.,
  • RMN, Social Work or Occupational Therapy QualificationCurrent registration with relevant professional bodyEvidence of post qualifying professional development in an acute setting.
