Full Job Description
The post holder will work within a multi-disciplinary home treatment team, sharing a base with CMHT, PCMHT, VCSE, peer workers and community groups. Core responsibilities will include:
- Comprehensive assessment of new referrals in crisis, intervention planning and care plan formulations.
- Care planning for patients who are accessing the on site beds.
- Service coordination duties include, understanding the roles of the team that day, which patients will be seen and identifying any gaps where services could be supported. Attending morning meetings/ referral meetings and Weekley MDT's
- Taking referrals from other services where the person is identified to be registered with a Heeley PCN GP.
- Carrying out initial HTT visit to set up plan of care and interventions for the team
- Link to other clinical areas such as inpatient and CMHT's
- Delivering supervision to more junior members of staff
- Contribute to service development

You will be working within the Urgent and Crisis and Home Treatment area of the Gleadless and Heeley Neighbourhood Mental Health Centre, working shifts of 8am - 8pm across 7 days per week on a rota basis. You will join an exciting new team, one of the first 6 pilot sites across the UK to launch to the Neighbourhood Mental Health Model of care. The aim is to establish a coherent, functionally integrated team, working with the VCSE, social care and the local community to deliver high quality mental health care for all the people of Gleadless & Heeley aged 18 or over.
