Band 6 Community Mental Health Practitioner

We have an exciting opportunity for a Band 6 Community Mental Health Practitioner to join our Adult Community Mental Health Team service, based at Chorley Health Centre, Chorley.

You will join a friendly team who are committed to their own development and who are, outgoing and sociable. The team is also flexible and family friendly where everyone's individualised cases are considered to help manage work-life balance where possible.

We are looking for a Core registered mental health practitioner, with post qualification experience in working clinically with service users experiencing a range of mental health problems, who are able to work as part of a multidisciplinary team, and has experience in assessing, planning, implementing and reviewing care needs and pathways with complex mental health issues.

We require someone who has great knowledge of medications and how to monitor them. Particularly psychotropic medications including Clozaril and Lithium.

You will also be required to have experience and knowledge of risk assessment and care and safety management, the ability to explore issues relating to the impact of health and social needs in mental health and well-being. We take a multi-disciplinary approach, in a holistic way.

You will also be expected to take part in our Duty system on a rotational basis as a back up duty officer.

Provide comprehensive assessment, advice and treatment to people with highly complex needs referred to Community Services in accordance with care pathways for given specialism

Deliver specialist interventions in accordance with care pathways for given specialism

Work closely with other professionals within and outside the Community Team

Actively supports the Team Leader/Advanced Practitioner in the achievement of KPI / CQUIN and related County Council targets, and maintenance of registration with regulating bodies

Take the lead on specific objectives as directed by the Team Lead and Clinical Team Lead

Provide supervision and direction to practitioners and support staff within community services, undertaking appraisals and supporting the personal development of colleagues

The post holder will have a positive attitude to the care of the service users, be adaptable and resourceful and will be required to work regularly without direct supervision.
