Working as part of a supportive multidisciplinary team, you will assess patients, plan and deliver individualised care, and support children and families throughout their healthcare journey. • Deliver safe, effective and childcentred nursing care using evidencebased practice.
• Assess, plan, implement and evaluate individualised care plans for children and young people.
• Carry out a range of essential nursing procedures and clinical assessments.
• Monitor patient condition, recognise clinical deterioration and escalate concerns appropriately.
• Administer medications safely in accordance with policies and guidelines.
• Maintain accurate, timely and comprehensive patient records.
• Work collaboratively with the multidisciplinary team to ensure coordinated care and safe discharge planning.
• Provide information, advice and support to children, young people and their families to promote health and wellbeing.
