Full Job Description
+ Deliver High-Quality Care: Support patients with neuro conditions, including stroke, to enhance independence and quality of life.
+ Collaborate Across Teams: Work closely with multidisciplinary colleagues to implement integrated care plans.
+ Professional Development: Engage with CLCH Academy programs for CPD, skill enhancement and clinical academic opportunities.
+ Mentorship & Team Support: Share knowledge, support colleagues and contribute to continuous learning within the team.
+ Community Visits: Provide care in the community as required. Car access is essential unless exempt under the Equality Act 2010, with appropriate insurance and documentation.
+ Record Keeping: Maintain accurate patient records and reports in line with professional and organisational standards.
Flexible Working: Participate in weekend work on a rota of one weekend every six weeks.
