Full Job Description

We are looking for a dedicated Band 5 Nurse to join our team within our new Healthy Ageing Hub, known as The Harbour, based in the Same Day Emergency Care (SDEC) unit at Newton Abbot Hospital.
You will be joining a service that provides compassionate, high-quality care primarily for older adults. Working as part of a supportive multidisciplinary team, you will play an important role in ensuring patients receive timely assessment, effective treatment, and safe, well-coordinated discharge.
In this Band 5 Nurse role, you will deliver holistic, patient-centred care from admission through to discharge, always striving to maintain the highest standards of treatment, dignity, and support for every individual you care for.
We welcome applications from newly qualified nurses, as well as 3rd year student nurses who are due to qualify. We are also open to sponsorship however, this is limited to applicants who already hold Skilled Worker status and are currently working in the UK as a NMC registered nurse.
Please note this is a full-time role 37.5 hours per week permanent opportunity, working Monday to Friday initially, with the potential for weekend working as the service develops.

+ assess, plan, implement, and evaluate individualised care, ensuring patients receive high-quality treatment throughout their journey,
+ carry out clinical observations, recognise and escalate health changes appropriately, and contribute to timely decision-making and discharge planning.
+ administer medications in line with policy, maintain accurate and contemporaneous records, and adhere to all trust guidelines, including infection prevention and control standards.
Elsewhere, you will support patients and their families, ensuring dignity, comfort, and clear communication is maintained at all times.

Working for our organisation
We are developing an exciting new model of care for people living with frailty, designed to keep patients well, independent, and supported in the place they call home wherever possible.
Based at The Harbour at Newton Abbot Hospital, this innovative service brings together community, acute, and specialist teams to provide joined-up, responsive care. Most patients will be triaged through a dedicated frailty coordination function and directed to the most appropriate part of the service based on their individual needs.
Where it is safe and appropriate, care will be delivered at home or in community settings, including intermediate care beds, supported by our Urgent Community Response (UCR) teams and Frailty Virtual Ward service. This allows patients to receive acute-level care outside of hospital, with access to wraparound support for both patients and their families/carers.
For those who need rapid assessment, the Same Day Emergency Care (SDEC) and ambulatory care service at Newton Abbot provides an alternative to hospital admission, with access to timely diagnostics and senior clinical opinion. This also supports patients already under virtual ward care who require urgent review.
When inpatient care is required, patients may be admitted to beds at Newton Abbot Hospital, with a proactive focus on safe and timely discharge back to their own home or usual place of residence as soon as possible., In this role, you will place people at the centre of everything you do, ensuring each individual's voice is heard and respected when planning and delivering their care.

+ You will build trusting relationships with patients and their families, taking time to listen, communicate effectively, and respond with empathy in every interaction. Care will be delivered in patients' own homes, always respecting their preferences, choices, and circumstances.
Working collaboratively within a multidisciplinary team, you will contribute to holistic, high-quality care planning, ensuring the best possible outcomes for patients and the wider community.
+ You will actively participate in MDT discussions, sharing insight, supporting decision-making, and contributing positively to service development and improvement.
+ You will take responsibility for enhancing services through feedback, reflection, and a proactive approach to change, helping to shape innovative ways of delivering care that support people to live well and independently for longer.
A key part of your role will include supporting and mentoring junior colleagues, fostering a positive team culture, and working together to achieve shared goals.
+ You will act as a professional role model, recognising the value and contribution of every team member and the wider support network involved in a patient's care.
+ You will also play an important role in maintaining a safe, supportive, and high-quality clinical environment for patients, carers, visitors, and staff, while consistently upholding Trust values and professional standards.
Through all aspects of your work, you will be encouraged and supported to develop your own clinical skills, engage in learning opportunities, and contribute to the continuous improvement of services.
