An exciting opportunity has arisen for an enthusiastic and forward-thinking Pharmacy Technician to join our Medicines Optimisation Team at Musgrove Park Hospital. As part of a growing multidisciplinary team of pharmacists, pharmacy technicians and assistants, you will play a key role in delivering safe, effective medicines management and optimisation across the hospital site. You will work closely with ward teams to ensure the highest standards of patient care, supporting safe and efficient use of medicines. If you are passionate about improving patient outcomes and want to develop your career within a progressive and dynamic team, we would love to hear from you.Main duties of the job In this role, you will support the delivery of safe, effective and patient-centred medicines optimisation across Musgrove Park Hospital. You will undertake medicines reconciliation on admission and contribute to accurate transfer of medicines information at discharge, ensuring continuity of care. Working closely with pharmacists, nursing staff and the wider multidisciplinary team, you will help identify and resolve medicines-related issues and promote the safe and appropriate use of medicines to improve patient outcomes. You will also provide advice and support to ward staff and patients on medicines management, including administration, storage and adherence. You will contribute to the effective use of electronic prescribing and medicines administration systems and support medicines safety initiatives, audits and quality improvement projects. Maintaining accurate documentation of clinical activity and contributing to service development will form an important part of the role. In addition, you will assist in the training and supervision of junior staff and students where appropriate, while maintaining your own professional development through ongoing training and education. You will play a key role in supporting a high-quality pharmacy service within a collaborative and forward-thinking team.
