- Passionate about providing excellent community-based nursing care
- Ready to work flexibly and independently as part of a multidisciplinary team
- Community nursing experience is helpful, but not essential--we'll support your development
- Able to travel across the locality with access to suitable transport, Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration, Demonstrate an understanding of the role of a community nurse
- Knowledge of legislation impacting on service delivery
- Knowledge of the modernisation agenda
- Demonstrate an understanding of evidence based practice
- Knowledge of local initiatives
- Demonstrate an understanding of the MDT
- Basic IT skills
- Knowledge and skills in the assessment process Desirable
- Able to mentor students
- Evidence of professional development
- Have a working knowledge and understanding of First Visit Criteria Experience Essential
- Basic health and safety awareness
- Experience of working within a healthcare setting
- Experience of working with families and carers
- Experience of working in a team
- Experience of working with computerised recording systems Desirable
- Experience of multi-agency working

- To have recently worked within a nursing capacity

We're looking for dynamic, forward-thinking Band 5 Community Staff Nurses to join our West locality Community Nursing Team within the Integrated Nursing and Conditions Services at CHCP CIC. We currently have a full-time position available. As part of our integrated community nursing team, you'll deliver high-quality, holistic care to people in their own homes, helping them stay well, safe, and supported in their community.
