  • Passionate about providing excellent community-based nursing care
  • Ready to work flexibly and independently as part of a multidisciplinary team
  • Community nursing experience is helpful, but not essential--we'll support your development
  • Able to travel across the locality with access to suitable transport, Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration, Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Knowledge of the modernisation agenda
  • Demonstrate an understanding of evidence based practice
  • Knowledge of local initiatives
  • Demonstrate an understanding of the MDT

    We're looking for dynamic, forward-thinking Band 5 Community Staff Nurses to join our West locality Community Nursing Team within the Integrated Nursing and Conditions Services at CHCP CIC. We currently have a full-time position available. As part of our integrated community nursing team, you'll deliver high-quality, holistic care to people in their own homes, helping them stay well, safe, and supported in their community.
