We are seeking enthusiastic Band 5 Community Staff Nurses to join our well-established Out of Hours Community Nursing Service, providing high-quality care across East Cheshire. No two shifts are the same, offering opportunities to develop your clinical skills, autonomy, and decision-making in a diverse community setting. Bring your nursing skills to the community, make a real difference when patients need it most, and become part of a service that truly values its staff., As a Band 5 Community Staff Nurse within the Out of Hours Community Nursing Service, you will provide high-quality, patient-centred nursing care to individuals in their own homes and residential care settings across East Cheshire.Working as part of a supportive multidisciplinary team, you will be responsible for assessing, planning, implementing and evaluating care, ensuring patients receive timely and effective nursing interventions during the out of hours period.,

• Delivering a wide range of community nursing interventions, including wound care, palliative and end of life care, catheter care and medication administration.
• Undertaking holistic patient assessments and using clinical judgement to identify changing healthcare needs.
• Supporting patients to remain safely at home, preventing unnecessary hospital admissions and facilitating early discharge where appropriate.
• Responding to urgent referrals and prioritising workload effectively to ensure safe and timely care delivery.
• Working collaboratively with patients, families, carers, GP services, acute hospitals, care homes and wider multidisciplinary teams.
• Maintaining accurate and contemporaneous clinical records in line with Trust policies and professional standards.
• Promoting health, wellbeing and patient independence through education and support.

• Current registration with NMC
• Evidence of recent professional development in an up-to-date portfolio, Recent previous experience within a comparable role/transferable skills
• Demonstrable knowledge of assessment and therapeutic interventions in area of practice
• Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
• Ability to prioritise and organise workload effectively
• Experience of using electronic patient/service user record systems
• Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e-mail or written communication
• Compassionate - with exceptional interpersonal skills with the ability to communicate effectively with staff, patients, carers and relatives (as appropriate) remaining sensitive and empathetic, Experience of mentoring students / supervise other junior staff
• Experience of working within a multi-disciplinary team.
• Previous experience delivering care in a community setting.
• Experience of working in a rehabilitation environment.
• Experience of working in multi-agency services. Experience of mentoring Knowledge and experience relevant to area of practice
• Understanding and / or experience of interagency working, Ability to assess clinical situations and act appropriately
• Able to assess, plan and evaluate patient care, considering the needs and desires of the patients
• Awareness of effects of ill health and the ageing process
• Awareness of the principles and practice of SSKIN.
• Computer literacy including Knowledge of Microsoft office systems and use of email, Leg ulcer assessment and management, Experience in palliative care, end of life care.
• Understanding of Gold Standard Framework
• Diabetes care/management.
• Understanding and/or experience of managing patients with LTC needs, Must be able to undertake all aspects of the role whilst working as a lone worker
• Able to organise own workload and deal with unpredictable nature of the role
• Ability to travel across the county with heavy / bulky equipment and the ability to travel from patient to patient, whilst maintaining a consistent caseload
