Job Introduction

Join our dedicated Community Nursing Team as a Band 5 Community Nurse and make a meaningful difference to people's lives every day. We have 2 positions available located at Warminster Hospital.

Main Responsibilities

• Participate in assessment and care planning of patient care in conjunction with the Team Manager and support of specialist team members.


• Implement and evaluate care and treatment in agreement with the patient and as defined in the care plan; escalate any deviation from the care plan or change in condition to the Team Manager /GP accordingly.


• Communicate effectively with patients, carers and all members of the multidisciplinary team.


• Prioritise daily work in line with clinical / patient need.


• Maintain accurate and legible patient records in line with professional and organisational requirements, including electronic patient record systems and paper records.


• Implement and work to ensure evidence-based care / best practice.


• Actively participate and contribute to team meetings and daily handover.


• Initiate health promotion activities with individuals, patients and carers.


• Actively involved in the development and implementation of community nursing service developments and initiatives.


• Participate in clinical audit and the implementation of changes to ways of working / clinical practice based on findings, recommendations and action plans.

The Ideal Candidate

You will be a NMC/1st level registered Nurse, with the ability to work both independently and as part of a wider multidisciplinary team. You should have strong communication and organisation skills, a flexible approach to deliver positive experiences to patients at every stage of their healthcare journey. You will have a full UK driving license with access to a car for work purposes with Business insurance for the vehicle used. Confident use of IT systems and working knowledge of Microsoft Office packages such as Outlook, Word, Excel are also required.
