Are you a dedicated and compassionate Registered Nurse looking to make a meaningful difference in peoples lives within their own homes? HCRG Care Group is looking for an enthusiastic Band 5 Community Nurse to join our dynamic District Nursing team. We have a full-time (37.5 hours per week) and a part-time (33.75 hours per week) vacancy available. This role forms part of a fantastic team transferring to a new organisation under TUPE on 1 July 2026, and if successful, you will transfer with the service., As a Band 5 Community Nurse, you will be an essential part of our 24 hour district nursing service, delivering high-quality, holistic care to patients in their own homes. Working under the direction of the District Nursing Team Manager/Leader, you will:

  • Carry delegated responsibility for the assessment, planning, implementation and evaluation of evidence based nursing care.
  • Provide skilled clinical care that supports patients to maintain independence and quality of life at home.
  • Supervise and support team members in the absence of the District Nursing Team Leader when required.
  • Work collaboratively with multidisciplinary colleagues to ensure safe, coordinated and person centred care.
