A vacancy has arisen for a Band 5 qualified practitioner to join South Glos. recovery north team at The Elms. Your role will involve keyworker for service user with serious mental illness in the community. We would welcome applications from newly qualified nurses and would offer a full preceptorship programme The team provides a range of key interventions which can include:

  • To provide assessment, formulation, treatment & therapeutic support for adults of working age with severe & enduring mental health and complex emotional needs.
  • Pharmacological interventions - medication management, prescribing and monitoring.
  • Psychosocial and psychological interventions.
  • Working in collaboration with AWP MDT and non AWP services.
  • Statutory work under the Mental Health Act and Mental Capacity Act.
  • Working in partnership to promote safeguarding of children and vulnerable adults and public protection.
  • Recognition of carer's needs, signposting and help in accessing other services.
  • Main duties of the job Applicants will need to demonstrate knowledge in the areas of the delivery of mental health care in the community, You will be required to provide key-working for an allocated caseload of service users with substantial and complex mental health needs. This will include key tasks of undertaking high quality assessments, developing care plans, delivering and reviewing therapeutic interventions. You will act as a supervisor and role model to others.

    Applicants will be expected to demonstrate a high level of decision making skills and to work independently. Applicants will also need to be proficient with the use of IT systems for both data and documentation purposes.
