A vacancy has arisen for Band 5 Community Mental Health Nurse - keyworker to join Swindon Recovery Team. The post will also be supporting depot clinic and working with service users to help deliver key interventions in order to stabilise and to supporting the step down to depot clinic - post will hold a key caseload in order to do this.
Applicants will need to demonstrate knowledge in the areas of the delivery of mental health care in the community, with particular reference to the needs of Older People, together with an understanding of both the PWP and Care Management processes.
You will be required to provide key interventions for an allocated caseload of service users with substantial and complex mental health needs. Potential additional interventions could involve being responsible for the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as key worker for a defined group of service users. Support will also be offered to family and carers., To manage a caseload of service users who are under the care of Swindon recovery team. To deliver key interventions primarily on psychosis pathway.
The clinician will need to assist in the running of the depot clinic including follow up of missed depots, keeping paperwork updated in a timely manner, following through additional intervention needs that have been identified as summarised above.
The clinician will help with transferring clients to appropriate services when needed including stepping down to primary or third sector services and GP's. The clinician will help in the ongoing development of this new service provision development.
Applicants will be expected to demonstrate a high level of decision making skills and to work independently. Applicants will also need to be proficient with the use of IT systems for both data and documentation purposes.
