Band 5 Clinical Pharmacy Technician - Out of Hospital Services

Are you an ambitious, patient-focused Pharmacy Technician looking to make a difference beyond the hospital setting?, University Hospitals Birmingham NHS Foundation Trust is seeking an enthusiastic Clinical Pharmacy Technician to join our Out of Hospital Services team at Solihull Hospital. You will work across intermediate care services in Solihull, supporting patients as they move between hospital, community and social care. As part of our medicine's optimisation team, you will help improve patient outcomes, support the safe and effective use of medicines, reduce waste and promote seamless pharmaceutical care. You will work closely with pharmacists, nursing teams, primary care, social care and community providers. This role is ideal for a Pharmacy Technician who enjoys working autonomously, building strong relationships and contributing to service improvement. We offer structured training, leadership opportunities and access to development programmes across UHB., As a Clinical Pharmacy Technician in the Out of Hospital Services team, you will provide medicines optimisation support to intermediate care services across Solihull, helping ensure safe, effective and high-quality pharmaceutical care.,

• Undertaking medicines optimisation monitoring visits.
• Supporting safe ordering, storage, handling, administration, recording and disposal of medicines.
• Working with pharmacists, nurses, social care, prescribing teams and community providers to improve patient outcomes.
• Identifying opportunities to improve medicines safety, reduce waste and support cost-effective prescribing.
• Investigating medication incidents and contributing to audits, quality improvement and service development.
• Producing clear visit reports, recommendations and action plans.
• Providing training, advice and support to healthcare and social care staff.
• Supporting patients with medicines-related needs, independence, understanding and safe transitions of care.
  
  You must be a registered Pharmacy Technician with an Accredited Checking qualification, significant pharmacy experience and excellent communication skills. You will be organised, proactive and able to work independently while building effective stakeholder relationships.
