Full Job Description

We are looking for a Patient Navigator to join the Diabetes & Endocrinology admin team who has administrative experience and to work as part of a team within a dedicated speciality/patient group. To ensure that all administrative functions for patients from the time of GP referral to the time of discharge are coordinated in line with specific care pathway instructions to facilitate the smooth running of the patients' journey. Where a patient has multiple specialist needs, assist the patient in coordinating their pathways to reduce multiple appointments across the Trust. Act as a single point of contact for patients within your remit. General administrative duties for the department such as filing of all correspondence including patient letters, results, photocopying and distribution of mail, managing mailboxes - identify and action any correspondence suitable for own response - produce relevant correspondence for clinician's reply.

We are looking for an administrative individual to join the Diabetes & Endocrinology admin team based at Chase Farm Hospital to oversee the patient pathway from referral to treatment. This involves the implementation of salient NHS targets, regular contact with patients and a keen attention to detail., You will have excellent written and communication skills - You will have administrative experience - You will have a good telephone manner - You will have sound organisational skills - You will be able to work both in a team environment and autonomously - You will be expected to input data on the Royal Free Hospital NHS system EPR - a patient records system and/or database system using excel and therefore excellent data entry skills are required., Education and professional qualifications Essential

  • English and Maths at GCSE level or equivalent
  • Desirable
  • Diploma level qualification or equivalent experience
  • RSA 2 or RSA 3 word processing/typing or equivalent experience
  • Knowledge of medical terminology
  • Courses / further study attended to demonstrate evidence of personal development
