Band 4 Macmillan Cancer Support Worker

A Vacancy at Barking Havering and Redbridge University Hospitals NHS Trust. An exciting opportunity has arisen to join the Head & Neck Cancer Team as a Band 4 Macmillan Cancer Support Worker. This rewarding role focuses on improving the experience of patients living with and beyond cancer. Working alongside Clinical Nurse Specialists and the wider multidisciplinary team, you will provide personalised, holistic support to patients and their families throughout their cancer journey. You will undertake Holistic Needs Assessments (eHNA), support personalised care planning, facilitate access to health and wellbeing services, and signpost patients to appropriate psychological, financial, community and voluntary sector support. You will act as an accessible point of contact, helping patients navigate their care and develop confidence in self-managing the impact of cancer and its treatment. We are looking for a compassionate, motivated and organised individual with excellent communication and interpersonal skills, who is passionate about delivering high-quality patient-centred care. Experience of working within a healthcare setting and supporting patients with complex needs is desirable. The Macmillan Head & Neck Cancer Support Worker will work as an integral member of the Head & Neck Cancer Multidisciplinary Team, providing personalised, holistic support to patients, families and carers throughout diagnosis, treatment, recovery and living with and beyond cancer. Working under the guidance of the Clinical Nurse Specialist (CNS) team, the post holder will support delivery of the NHS Personalised Cancer Care agenda through Holistic Needs Assessments (eHNA), personalised care and support planning, health and wellbeing interventions, emotional support, signposting and care navigation. The post holder will act as an accessible point of contact for patients with non-complex needs, supporting them to access the right information, support and services at the right time, while promoting self-management and independence wherever appropriate. The post contributes directly to improving patient experience, reducing health inequalities and supporting people affected by Head & Neck cancer to manage the physical, emotional, practical and social impact of their diagnosis and treatment. We run a Women's Health Hub in Ilford; an Ageing Well Centre in Hornchurch; and Community Diagnostic Centres (CDC) at Barking Community Hospital and at St George's Health and Wellbeing Hub. These CDCs are open 12 hours a day, 7 days a week.
