Full Job Description

As a Band 3 Health Care Assistant in the District Nursing team, you will support the delivery of safe, high-quality care to patients in their own homes. Your main responsibilities will include:

  • Providing person-centred care in patients' homes, working within your role and competencies
  • Supporting people with long-term conditions to maintain their independence and wellbeing
  • Working closely with registered nurses and other colleagues to meet individual patient needs
  • Contributing to a respectful, inclusive and high-quality community nursing service
  • Communicating effectively with patients, carers and colleagues

    We are looking for a compassionate and motivated Health Care Assistant who values dignity, diversity and holistic care.
  • To succeed in this role, you will need:
  • NVQ Level 3 in Health and Social Care
  • Relevant experience in an acute or community healthcare setting
  • NHS experience
  • The ability to work effectively as part of a team and support people with a range of care needs
  • Community healthcare experience is desirable, and training will be provided to support your development. You will join a supportive and collaborative team with opportunities to learn, develop your skills and progress your career. Patients are visited locally anda driving licence is not required.
