Full Job Description

An exciting opportunity has arisen for an Assistant Practitioner to join our Suffolk Integrated Stroke Service. As part of the Stroke Early Supported Discharge Team, you will work alongside therapists and other multidisciplinary colleagues to deliver evidence-based rehabilitation to stroke survivors in their own homes and communities. The post holder will manage a delegated caseload, supporting assessment, treatment and ongoing review of patients with non-complexneeds, while contributing to six-month stroke reviews, service development and SSNAP data collection. This is an excellent opportunity to develop specialist rehabilitation skills within a supportive and innovative stroke service., o Provide skilled rehabilitation support within the Stroke Early Supported Discharge (ESD) Team, delivering assessment, treatment and progression of evidence-based rehabilitation programmes for stroke patients with non-complex needs.

o Work independently with delegated patients under the supervision of registered therapists, monitoring outcomes and adapting treatment plans in line with agreed protocols and person-centred goals.

o Support patients to maximise functional independence using evidence-based interventions that reflect their individual abilities, preferences and cultural needs.

o Contribute to the delivery of six-month post-stroke reviews and maintain accurate clinical and audit data, including inputting information to the Sentinel Stroke National Audit Programme (SSNAP) database.

o Provide support to therapists, supervise students where appropriate, and participate in the delivery of a seven-day service.
