Associate Practitioner

As a Band 4 Associate Practitioner, you will play a key role in supporting patients through their rehabilitation journey, working within the Garden City Team at Gravesham Community Hospital. This role is ideal for experienced Rehabilitation Assistants, Therapy Assistants, or Healthcare Assistants eager to advance their careers. You will support high-quality rehabilitation services, such as pulmonary rehabilitation programmes and community-based interventions, and individual patient support. Collaborate with a passionate multidisciplinary team to deliver personalized care, contributing to the development and improvement of community services.
