Full Job Description

This role is to support the planned care pathway within the team.

The successful candidate will have an opportunity to be involved in group delivery and coordination.

The successful candidate will support the physical care pathway withing the team and will need to complete physical health checks for people on caseload, including ECGS, venepuncture etc.

We will provide you with clinical caseload and managerial supervision, a thorough induction and an extensive range of training opportunities relevant to the role.

This post is not open to trainees therefore the successful individual will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited into this Band 4 position.
