Associate Advanced Nurse Practitioner

We are seeking a compassionate, highly motivated, and skilled Associate Advanced Nurse Practitioner to join our dedicated hospice community team. This is an exciting opportunity to provide expert, person-centred care to patients with palliative and end-of-life care needs in their own homes and community settings. Working collaboratively with multidisciplinary colleagues, you will play a key role in supporting patients and their families, promoting quality of life, symptom management, and dignity throughout their care journey. Bank - zero hours contract. A clean driving licence and use of a vehicle with business insurance is essential. Hours a week to be worked flexibly to include days, nights, weekends and bank holidays on a shift rota basis Please note: we are not part of the NHS and regrettably we do not hold a licence to enable visa sponsorship, The Associate Advanced Nurse Practitioner - Community role will contribute to the provision of specialist palliative and end of life care in the community, being based at Mountbatten Hampshire. Under the supervision of the Locality Leads, they will work autonomously and as a member of the Community Palliative Care Team influencing the delivery of ongoing patient care, as well as providing advice and support to patients, their families and other healthcare professions within the community., Key Tasks: To provide an environment that encourages patients and their families to play an active part in their care enabling them to voice their wishes about current and future care requirements To provide advanced levels of clinical decision-making through assessment and interpretation of clinical information to support patients, families and carers who have complex or rapidly changing physical symptoms, emotional or practical needs To assess, plan, implement and evaluate patients care needs as required by your designated patients, including admittance to the Hospice/Hospital and advanced care planning including identifying their preferred place of death To provide appropriate pain and symptom management and provide directional support for their psychological and spiritual needs of the patient, their families, and carers To provide support, information and advice to patients, families, colleagues, and other healthcare professionals, including supporting the Mountbatten Care Co-Ordination Centre Prioritise daily duties in response to patients and familys requirements in a manner that maintains and promotes quality, NMC code of practice, the patients care needs and the values of Mountbatten To be fully conversant with organisational medicines policies and palliative care guidelines, algorithms and administer medication in line with these controls procedures To provide training support in the use of syringe drivers and provide teaching to others including, staff nurses and students in collaboration with the Community Team and other professionals In accordance with Mountbatten Infection Control policy and procedures, promote an environment that supports infection prevention and control policies and procedures, ensuring that they and other staff practice in accordance with these documents The postholder will act as an advocate for their patients and families ensuring their views are known to the team including in the case of advance care planning and occasions where offered care is declined

• Able to work as part of a team
• Able to be an effective advocate for the patient with multi-disciplinary teams across all healthcare settings
• Able to work shift patterns on a rota basis including nights, weekends, bank holidays, First Level Registration with NMC and in possession of a relevant degree in Health/Nursing Studies
• Full UK Driving Licence (clean) and use of vehicle with Business Insurance, Experience of working collaboratively within multidisciplinary teams and the ability to form good working relationships at all levels including volunteers
• Demonstrable experience of caring for vulnerable adults in a care/hospital or hospice setting
• 2 years post registration clinical practice experience

£19.79 to £23.82 an hour plus enhancements (Mountbatten Band 6)
