Full Job Description
This role focuses on community engagement, delivering interventions, and providing education to care home staff and residents, alongside completing multi-factorial falls assessments and interventions.
The post holder will support the Falls and Frailty teams in delivering high-quality, person-centred care for individuals living with frailty. Key responsibilities include supporting frailty management across community settings, delivering falls prevention and frailty-focused rehabilitation, and contributing to personalised care plans that promote strength, balance, mobility, and independence. Strong emphasis is placed on early intervention and prevention to reduce hospital admissions and enable individuals to remain safely in their preferred environment.
Working collaboratively with the Falls Clinical Specialist, Frailty AHPs, and wider multidisciplinary team (including GPs, Occupational Therapists, Physiotherapists, Nurses, Social Care, and third sector staff), the role supports a coordinated pathway for frailty and falls management.
We are looking for experienced and dedicated individuals with a strong interest in falls prevention and rehabilitation of our frail older population, including those living with frailty, Applicants should have relevant community experience, strong communication, and the ability to work independently and within a team. Flexibility, excellent organisational skills, and participation in a 7-day rota are essential. A driving licence and access to a vehicle are required
