Full Job Description

The Initial Response Team in Adult Social Care is looking for an enthusiastic and highly motivated Assistant Social Care Practitioner to support the increased demand within the team. The focus of the Initial Response Team is to support prevention, by having strengths based conversations at the point of contact so that we ensure the right care at the right time making the customer journey a positive experience, and ensuring that our customers remain independent. Working with adults, their families and carers you will need relevant Level 3 qualification plus experience of working with vulnerable groups and evidence of continuous professional development in a related capacity . In addition to a significant understanding of the social care needs of older people and vulnerable adults. We also complete a Rapid Response function undertaking visits to those in most urgent need and work alongside partner agencies including health, housing and police.
