Full Job Description
The Healthy Family Team work in partnership with other agencies to deliver care and interventions to children, young people and families. The team is led by the Specialist Public Health Practitioners (SPHP), with support from the skill mix colleagues in working with 0-19 year children and young people within a defined geographic area. Working with under age 5 will include breastfeeding support, completing mandated developmental reviews, and supporting the team to meet their Key Performance Indicators. The interventions for school aged children include supporting children and young people aged 5-19 with emotional health and well being needs in schools and other community settings. Other duties will include attending safeguarding meetings for the SPHP with Social Care and others meetings as delegated by the SPHP., see Job Description document: You will be responsible for supporting the team to ensure that the needs of the clients are met in line with the Service Delivery Framework. You will work directly with children and their families to deliver short pieces of work to meet their needs. You will be responsible for onward referrals to specialist services as required. You will complete mandated reviews to identify any children that are not meeting their developmental milestones and liaise with senior staff for guidance with regards to episodes of care. You will be trained to deputise for the SPHPs as required at safeguarding and other meetings. We offer full training, a great induction and a very friendly environment for the right candidate. We are currently working to a hybrid model, out and about as service needs require, working from home as and when appropriate, appropriate equipment for working at home will be provided. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary / skill level is below the minimum required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa. NVQ level 3 in Early years or Childcare or equivalent (BTEC, NNEB, CACHE Diploma)or Nursing Associate with previous experience of working with children and young people. Experience Essential - Experience working with families within a childcare setting
