Full Job Description

Key Responsibilities: Be responsible for a delegated caseload following initial assessment by the registered staff, carrying out interventions, including goal setting, outcomes, risk assessment & discharge. To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling. To be responsible for providing accurate records of information required by the Trust for audit purposes. To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register. To supervise pre-registration students, Band 3 and 2 and unregistered staff. To employ professional curiosity in your day-to-day role and act upon any findings appropriately. Contribute to the team/service development with the assistance of clinicians and Team Lead Participate in team meetings to ensure effective exchange of information within the
multidisciplinary team for optimum care/goal planning and coordination of care delivery Build relationships to promote seamless care Maintain accurate record keeping Participate in clinical audit and service developments, Work within an multidisciplinary framework, undertaking a key-working role for non complex patients as triaged by registered staff Liaise closely with medical, nursing, allied health professional, social care, acute and voluntary sector staff to promote greater understanding of the Falls Prevention Service Implement, evaluate and modify intervention as directed Monitor progress, taking account of the environment and adjust own clinical interventions accordingly. Undertake risk assessments as part of everyday clinical practice, complete relevant documentation and seek guidance as necessary Follow the treatment plan and feedback on progress and highlight need for re assessment when needed. Contribute to delegated assessments Plan home visits and case conferences as directed Once a programme is established, hand over patients as appropriate to other multidisciplinary team members / support
