Full Job Description

We have exciting opportunities for a Band 4 Assistant Practitioners to join the Community Childrens Nursing Service. The role will support a Child with high complex physical needs following safe delegation and with guidance from a Registered Nurse within the school environment., The Assistant Practitioner will work under the direction of the registered nurse to deliver high quality, holistic, compassionate health care to a Child within the school environment with high complex needs, providing the required support for the child and family. The post holder is expected to independently manage their own work and case load, undertaking tasks delegated by a registered practitioner with appropriate supervision in place. Having an understanding of evidence based practice and delivering care in line with current evidence, the post holder will take responsibility for taking action relative to an individual's health and care in accordance with organisational policy and procedures

The post holder will be responsible for some elements of assessment, implementing programmes of care and modifying individualised care plans, reporting back to the registered practitioner. They may delegate work to others and may supervise, teach and assess other staff.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
