Full Job Description
The Aylsham Medical Day Unit is a nurse-led unit providing planned care for patients referred from all specialties across the Acute Trust. Established in 2018, our service treated more than 11,000 patients last year alone. Our patients are at the heart of everything we do. We aim to support patients living with chronic or acute conditions to remain in their own homes, avoid unnecessary hospital admissions, and facilitate early discharge from inpatient care.We are a dedicated and highly skilled team, committed to delivering the highest standard of care to our patients. AMDU is a unique and dynamic environment, where our service continually evolves through the development ofnew pathways designed to meet the changing needs of our patients.We are looking for an enthusiastic and motivated Assistant Practitioner to join our friendly established team. You will work independently under guidance and direction of registered Nurses. The successful candidate will be passionate about delivering high-quality patient care and be keen to develop their clinical skills. An interest in service improvement, evidence-based practice, and supporting positive patient outcomes is essential.The unit operates 7am until 19:30, 7 days a week. This post is part time (28.5h) worked as three 9.5 hour shifts., The Assistant Practitioner will work as part of the nurse led team within the Aylsham Medical Day Unit, providing high-quality, patient-centred care for patients attending for planned day procedures and treatments. The post holder will play a key role in promoting positive patient experiences, delivering safe and effective clinical care and supporting patient flow.Key duties will include:
- Undertaking therapeutic venesection in line with Trust policy and competency requirements for patients with haematological conditions such as Haemochromatosis and Polycythaemia Vera.
- Providing care and support to patients attending the unit for day procedures, investigations, and treatments.
- Performing clinical observations and escalating any concerns regarding a patient's condition appropriately.
- Assisting with the preparation, monitoring, and aftercare of patients undergoing procedures and treatments.
- Supporting patients throughout their treatment journey by providing reassurance, information, and health education where appropriate.
- Maintaining accurate and timely patient documentation in line with Trust policy and professional standards.
- Working collaboratively with the multidisciplinary team to ensure safe, effective, and efficient patient care.
- Maintaining a safe, clean, and organised clinical environment in accordance with infection prevention and control standards.
- Participating in audit, service improvement, and ongoing professional development activities.
