Full Job Description
Community Mental Health Services across NLFT are transforming. The Assistant Practitioner will work within the Intensive Outreach (Assertive Outreach) Pathway embedded in the multi-disciplinary Core Team to proactively engage adults with psychosis and/or complex needs who find it difficult to access traditional services. The post holder will deliver recovery-focused, trauma-informed and culturally competent interventions under supervision, contributing to assessment and ongoing care using Dialog+ care planning and digital tools including RiO and MAST. The role involves assertive community outreach, lone working in line with policy, multi-agency collaboration (VCS, primary care, local authority) and supporting safe step-up/step-down decision making. Deliver recovery-focused, trauma-informed interventions under supervision to support engagement and social inclusion.
- Support assessment processes including data collection, monitoring and contribution to Dialog+ care planning.
- Undertake assertive outreach including home visits; adhere to Lone Working Policy and use Trust devices.
- Observe and report changes in mental and physical health and escalate risks appropriately.
- Support psycho-social and therapeutic group/individual interventions.
- Support physical health checks and health promotion within competence.
- Maintain accurate RiO documentation and use MAST to support caseload oversight.
- Work with carers where appropriate and contribute to carer assessments.
- Participate in MDT discussions regarding discharge, transition and admission avoidance.
- Respond appropriately to behaviour that challenges using approved techniques.
