Full Job Description

The Reablement Team assists patients requiring additional support and rehabilitation on discharge from hospital, and those in the community who need increased care or therapy input. Interventions and assessments are holistic, person centred, and focused on maximising independence. Patients are at the heart of everything we do. This is an exciting opportunity to work under the guidance of qualified therapists whilst working closely with other disciplines, gaining knowledge and experience across a variety of specialities. As a valued member of the reablement team, you will work closely with health and social care team members, having the unique opportunity to experience both sectors. Working under the guidance of qualified therapists, you will deliver person-centred interventions and support service users in following tailored reablement programmes. You will manage a caseload of appropriate reablement cases, regularly reviewing progress, adapting support plans where appropriate, and maintaining a strong focus on achieving positive outcomes. You will be part of a supportive multidisciplinary team that includes physiotherapists, occupational therapists, nurses, therapy assistants and social care colleagues. Through collaborative working, you will help ensure that each individual's needs are met through a coordinated and holistic approach. Regular clinical supervision and professional support are provided. Flexible working arrangements considered.
